Purpose The purpose of the STRIVE (Special Needs) Program is to provide a structured, supportive environment for residents who need specialized services. The therapeutic focus of this program is to assist the residents in gaining skills to manage their mental health needs in conjunction with successful life skills. The goal of this program is for the resident to come to a greater understanding of their strengths and abilities and to then develop new skills /tools for effective coping and constructive behaviors. The goal of treatment is to prepare the resident to succeed in a lest restrictive level of care. Admission Criteria
Referrals are made via the Admissions Department following the referral process outlined in El Pueblo’s written plan of service Discharge Criteria
Provisions of CareResidents receive 6 hours of educational programming each day during the school year; recreation/leisure activity of one hour daily with an additional 3 hours per week of specialized recreation services; a minimum of 3 hours per week of group counseling; a minimum of 1 hour per week of individual therapy; a minimum of 2 hours of family therapy per month; 1 to 3 staff ratio; 24 hour structured therapeutic milieu services; referral for specialized or medical services and case management as needed. Clinical Services Case Management/ Mental Health TherapyEach resident is assigned a primary MA or PhD licensed therapist. The primary therapist provides coordination of services; ensures that case management services are appropriate for the resident’s needs; prepares the master service plan and monthly reviews of the plan; develops and reviews Individual Crisis Management Plans and behavior plans; and provides individual and family therapy. Residents are assigned by the primary therapist to therapy groups based upon individual needs and service plan.The primary therapy leads the weekly Rehabilitative Services meeting and provides guidance to milieu staff.Medical and Psychiatric ServicesPhysical, Vision and Dental services are provided by community Medicaid providers. A sick call process is utilized by nursing to determine need for referral to community medical providers for injury or illness. El Pueblo’s consulting Psychiatrist provides medication management services and sees the resident on a monthly basis for ongoing psychiatric services. On a bi-weekly basis, the Nursing Department shall provide Health Education and Support classes. Occupational, Physical and Speech TherapyResidents in need of occupational, physical or speech therapy shall be seen by the District 60 therapists. El Pueblo staff follow recommendations of these specialists as to ongoing interventions and activities provided on campus. Milieu Services The therapeutic milieu provides a higher level of supervision with a staffing ration of 1 staff to 3 residents. The staff in this program have additional training specific to working with this population. The staff supervise, structure and participate in activities that are recommended by an outpatient Occupational Therapist and a Therapeutic Recreation Specialist. The daily routine is broken down into time increments designed to prevent problems with attention and to provide a variety of activities throughout the day. Residents have the opportunity to practice life skills of cooking, cleaning, laundry, and leisure interests in the cottage. Recreation and LeisureResidents will have organized staff-directed and supervised recreation activities for at least one hour a day. Part of the recreation schedule will include three hours per week of instruction from a recreation specialist who is under the supervision of a Certified Therapeutic Recreation Specialist. El Pueblo provides a stimulation/skill building program designed to promote the independence and self-care capabilities of the residents. The stimulation/skill building program is designed to service the residents in the Special Needs Program. Recreation specialists conduct group activities that are designed to address teamwork, trust, behavior, motor functioning, intellectual functioning, perceptual skills and community reintegration. Education ServicesResidents receive individual and small group educational services in the classroom, led by a special education teacher and assisted by paraprofessional(s). They participate in educational activities provided by the El Pueblo School. Services are provided as specified by the student’s Individualized Education Plan (IEP). The goal of the Education Program is to:
Curriculum and Instructional Design Academics: Small group and individual instruction as determined by the students’ IEP and includes core academic areas of Reading, Written Language, Math, Social Studies, Science and PE. Academics will be predicated on functional skills, particularly community access training and activities of daily living. Life Skills: Community-based activities. Also, students participate in daily living skills such as personal hygiene, cooking and housekeeping.
