Admission Criteria

The following criteria must be met for the Community to Community Program:

          ·        Must be between the ages of 10-18 years

·        Must be willing to participate in the Community to Community Program

·        Must be referred by contracting entity (DHS, DYC, Courts, Schools or Private)

·        Agree to adhere to all applicable policies and procedures of El Pueblo

Referral Process

  1. For youth already enrolled in one or more El Pueblo services, Wraparound coordinator will be invited to a staffing to discuss transition to Community to Community program.
  2. New referrals may be made to Wraparound coordinator at (719) 404-1102.
  3. A Referral Packet will be required, which consists of the following:

·        Documentation of current issues that bring about the need for Community to Community services

·        Family and Social History to include:

    1. previous treatment and/or placements, if applicable
    2. previous response to treatment, if applicable

·        Copies of the youth’s birth certificate, social security card, immunization record and school records

  1. Community to Community team will review the material, and pursue a contractual agreement to fund services if family is agreeable based on the individualized level of services desired.
