Purpose The purpose of the STRIVE II Independent Living Program is to provide Residents enter into the STRIVE II independent living program when it is determined that the residents are in need of preparation for discharge to a less restrictive setting, or home with family members. The emphasis is on developing the ability to be responsible for self, work with others constructively and have exposure to a real life work environment. While in this program the resident will gain a greater understanding of their strengths and abilities and to then develop new skills /tools for effective coping and constructive behaviors. The goal of treatment is to prepare the resident to succeed in the lowest level of care once in the community. All El Pueblo policies and procedures are applicable to this dimension of the El Pueblo Performance Program for Youth© Population Served
Admission Criteria
Referrals are made via the Admissions Department following the referral process outlined in El Pueblo’s written plan of service Discharge Criteria
Provisions of CareClinical ServicesCase ManagementA master’s level clinician who will develop the plan of service provides case management. The case manager provides coordination of services; ensures that case management services are appropriate for the resident’s needs; prepares the master service plan and monthly reviews of the plan; develops and reviews Individual Crisis Management Plans and behavior plans if needed; and directs the provision of counseling and skills training provided by direct care staff. Medical and Psychiatric Services Physical, Vision and Dental services are provided by community Medicaid providers. A sick call process is utilized by nursing to determine need for referral to community medical providers for injury or illness. El Pueblo’s consulting Psychiatrist provides medication management services and sees the resident on a monthly basis for ongoing psychiatric services. Milieu Services The therapeutic milieu provides a level of supervision with a staffing ration of 1 staff to 4 residents. The staff in this program have additional training specific to working with this population. The staff supervises the resident and facilitates the resident’s participation in therapeutic activities to assist the resident in development of pro-social and constructive leisure activities. The daily routine is broken down into time increments designed to prevent problems with attention and to provide a variety of activities throughout the day. Residents have the opportunity to practice life skills of cooking, cleaning, laundry, budgeting and leisure interests in the cottage. Vocational Opportunities and/or Work Residents will adhere to the El Pueblo dress code when working with our vocational partners. Resident will abide by the requirements for their particular job while at an off campus work site. When a resident is accepted into the vocational training program, an El Pueblo staff will accompany the resident for the initial 30-day training period. When the resident is deemed appropriate to be supervised by the work site staff, El Pueblo staff will only provide transportation. However, the resident may have increased supervision from El Pueblo staff if requested by the vocational training program partner. Regular communication between the work site staff and El Pueblo staff to help ensure the resident’s success may occur in formal staffings and in brief conferences. One goal of this program is to help the residents prepare and plan for their future living expenses. Recreation and LeisureResidents will have organized staff-directed and supervised recreation activities for at least one hour a day. Part of the recreation schedule will include three hours per week of instruction from a recreation specialist who is under the supervision of a Certified Therapeutic Recreation Specialist. El Pueblo provides a stimulation/skill building program designed to promote the independence and self-care capabilities of the residents. The stimulation/skill building program is designed to service the residents in the STRIVE Program. Recreation specialists conduct group activities that are designed to address teamwork, trust, behavior, motor functioning, intellectual functioning, perceptual skills and community reintegration. Education Services Individualized based on residents needs and IEP (Individualized Education Plan).
